SUBJECT:	REVISED PLAN REVISION DISTRIBUTION FORM

Effective immediately, use the attached form for distribution of plan revisions.

Insert a brief discussion between the first and last paragraphs of the form letter, including the following information:

· Name of the individual who revealed the need for the revision
· Reason for the revision
· Description of the revision

Consult with the Bridge Construction Engineer to determine if any resulting plan quantity change should be addressed in the revision letter.

For revisions to plans that require final plan submittal to the FHWA, include the FHWA administrator in the carbon copy list.
