Recertification Process 2011:
All “Approved” and “Approved for Provisional Use” products must recertify annually.
Download and save the PEP Recertification Form:
click here.
Fill out the PEP Recertification Form accordingly and save it. The form can be saved in Adobe
Reader. NOTE: Handwritten forms submitted will NOT be accepted.
Fill out one form per product. Multiple products submitted on one from will not be accepted.
E-mail the Recertification Form(s) to
productevaluation@ncdot.gov
by January 31, 2012.
NOTE: Hard copy Recertification Form submissions will NOT be accepted.
If a Recertification Form is not received by January 31, 2012, the product will be removed from
NCDOT’s APL.

Please note, you may only submit ONE product per PEP Application form. If you would like
to submit multiple products, you must fill out a PEP Application for EACH product.
